HIGH DEDUCTIBLE HEALTH PLAN (HDHP) - In & Out of Network
The High Deductible Health Plan (HDHP) is designed to provide the benefits of a PPO plan, plus an option to enjoy the added value of a tax advantaged Health Savings Account (HSA).
A HDHP and HSA work together to provide you with the medical coverage you need and an account to help pay for what the plan does not cover. You can make pre-tax contributions into your HSA account, and the University will also make contributions into your account based on your enrollment tier. Health Savings Account (HSA) Employer Contributions:
Employee Only Plan: $1,500/year ($62.50 semimonthly)
Employee + Spouse: $2,500/year ($104.17 semimonthly)
Employee + Child(ren): $2,500/year ($104.17 semimonthly)
Employee + Family: $3,000/year ($125.00 semimonthly)
EXCLUSIVE PROVIDER ORGANIZATION (EPO) - In Network only
The Exclusive Provider Organization (EPO) plan gives you the freedom to see any Network Physician or healthcare professional, including specialists, without a referral or needing to designate a Primary Care Physician.
Much like an HMO plan, the EPO plan does not allow for “out-of-network” charges; it is the members’ responsibility to be aware of the plan network and from whom they receive care.
PREFERRED PROVIDER ORGANIZATION (PPO) - In & Out of Network
The Preferred Provider Organization (PPO) plan gives you the freedom to see any Network Physician or healthcare professional, including specialists, without a referral. With the PPO plan, you can seek care with an In-Network or Non Network Provider. You will receive the highest level of benefits when you seek care from a Network Provider.
Care received from a Non-Network physician, healthcare professional or facility means higher out-of-pocket cost. If you choose to seek care outside of the Network, the reimbursements to providers have not been pre-negotiated and you will be responsible for any fees greater than the usual and customary rate for the service.
Your Blue Shield insurance card will be mailed to you within 10 business days after your benefit enrollment is approved in Workday. If you do not receive your card, follow these steps:
Having a Primary Care Physician (PCP) ensures consistency and efficiency in your healthcare. Finding a PCP that meets your needs and accepts your insurance is key to managing your care.
