- Notify the Benefits Department of absences lasting greater than five (5) continuous workdays, so that a determination can be made regarding eligibility for leave, and to identify any required supporting documentation*

Reasons for Leave:
- Your pregnancy-related disability
- Parental leave
- Your own serious health condition
- The care of a family member due to their own serious health condition
- Assisting a qualifying family member who has a “qualified exigency” related to active duty or call to active duty with the Armed Forces
- The care of a qualifying family member who is a covered service member of the United States Armed Forces who has a serious injury or illness in the line of duty while on active duty, or is a veteran of the Armed Forces including the National guard and Reserves at any time within 5 years preceding treatment for a serious injury of illness
- Other reasons may apply. Please contact the Benefits Department to discuss additional options
*Requesting a Reasonable Accommodation: Should you need to request an accommodation, please have your health care provider complete the Request for Reasonable Accommodations form. While we do not need a diagnosis, your health care provider will need to explain how your limitations (temporary or permanent) impair your ability to perform the duties of your job and provides recommendations of specific reasonable accommodations
