PPO Plan is a nationwide preferred provider organization (PPO) plan offered by Delta Dental, one of the nation’s largest and most experienced dental benefits carriers. 

With the PPO plan, you have the flexibility to choose any licensed dentist. However, utilizing a PPO network dentist can help you save costs. Please review the DPPO Benefit Summary for more details.

DeltaCare USA (DHMO) 

DeltaCare USA (DHMO) is offered by Delta Dental affiliate PMI Dental Health Plan. With the DHMO plan, each participant selects their own Primary Care Dentist (PCD), who will direct all of their care. Please review the DHMO Benefit Summary for more details.

This plan is only available in Arizona, California, Washington, and Nevada.
